Subscription plan request form

Tick to confirm that you understand and accept the following statement. You do not have to book a treatment or make a purchase every month. The money paid to Opalesce Skin Clinic through your subscription will build up and be redeemed against future treatments or purchases along with your membership discount.

Our subscription will continue until you choose to cancel it. There is no cancellation fee. You can cancel at any time by cancelling the direct debit with your bank (instant), or by submitting a cancellation request via email to Opalesce Skin Clinic (please allow up to 5 working days for processing).

On cancellation of your membership plan, any funds not already redeemed in the clinic are non-refundable but will remain on your account to be used towards future treatments or products.

Opalesce Skin Clinic reserves the right to amend membership discounts from time to time.

You consent to receiving an email from Opalesce Skin Clinic with a link to the subscription provider, GoCardless, in order to set up your direct debit for subscription payments.

You understand that your subscription will not become active until you have followed the link in the email and completed the direct debit setup. Once completed, GoCardless will collect your first payment via direct debit approximately 5 days after setup.
